Results Show Diabetes Management Program is Helping People Take Good Care

Afni partnered last year with Blue Cross and Blue Shield of Illinois, local physicians, and other area companies to launch a program that helps people manage their diabetes. Called "Diabetes Checks and Balances," it has helped successfully decrease emergency room visits and hospital admissions among participants. |
